Management of Change is mandatory in today's regulated operating environments.

From current P&ID's, to operating manuals, to routine operating changes, there must be an "official" documented evaluation process prior to implementing change.

This could be a HAZOP or technical review by appropriate knowledgeable personnel, usually including third party individuals for "open" thinking.
